Fatigue is more than feeling sleepy after a busy week. When low energy lasts for several weeks, affects concentration, makes routine tasks feel difficult, or does not improve with rest, it deserves a closer look. An example workup for persistent fatigue shows how a primary care clinician can move beyond a single lab test and look at the full picture of your health.
The goal is not to assume there is one cause or to order every test at once. Fatigue can stem from sleep quality, stress, medications, nutrition, infections, hormone changes, or medical conditions that need ongoing attention. Why persistent fatigue needs a personalized evaluation
Everyone uses the word “fatigue” differently. One person means they fall asleep during the day. Those differences matter because they point the evaluation in different directions.
Your clinician will also consider timing. Did the fatigue begin suddenly after an illness? Has it gradually developed over months? Is it worse in the morning, after meals, during physical activity, or at certain times of the month? A clear pattern can be as useful as a laboratory result.

Supplements can be helpful in the right setting, but they may also affect lab results or interact with medications. Bring an up-to-date list, including dosages, whenever possible.
Your medical and family history helps set priorities. Conditions such as diabetes, thyroid disease, kidney disease, anemia, heart or lung concerns, and depression can all be associated with fatigue. For some patients, recent dietary changes, heavy menstrual bleeding, gastrointestinal symptoms, or a restrictive eating pattern may suggest a nutritional cause.
The physical exam is not an afterthought. A clinician may check blood pressure, heart rate, weight trends, oxygen level, thyroid area, heart and lung sounds, skin color, swelling, and signs of dehydration. Depending on your symptoms, the exam may also include a focused neurologic or abdominal assessment. These findings help determine whether testing should be routine, more targeted, or completed urgently.
Common tests and why they may be ordered
There is no universal fatigue panel that is right for every patient. Testing should follow your history and exam. Still, an initial evaluation often includes a few common studies that can reveal treatable causes:
- A complete blood count can look for anemia, signs of infection, and changes in blood cells.
- A comprehensive metabolic panel can assess kidney and liver function, electrolytes, blood sugar, and other metabolic markers.
- Thyroid testing, often including TSH and sometimes additional thyroid levels, can help evaluate whether thyroid function is contributing to symptoms.
- Iron studies, vitamin B12, vitamin D, or other nutrition-related tests may be appropriate when your symptoms, diet, or health history suggest a deficiency.
Your provider may also recommend an A1C to evaluate longer-term blood sugar patterns, a urinalysis when symptoms point in that direction, or tests related to inflammation or infection when clinically appropriate. Pregnancy testing may be discussed for patients who could be pregnant. The right choice depends on the person in front of the clinician, not a checklist alone.
More testing is not always better. Broad testing can uncover findings that are unrelated to fatigue and create unnecessary worry or follow-up. When sleep should be part of the workup
A patient may spend eight hours in bed and still have poor-quality sleep. Loud snoring, witnessed pauses in breathing, waking with headaches, dry mouth, restless sleep, or falling asleep unintentionally during the day can raise concern for a sleep disorder. In those situations, a sleep study or referral may be more useful than repeating vitamin tests.
Sleep can also be affected by irregular schedules, caffeine late in the day, alcohol near bedtime, reflux, anxiety, or frequent nighttime urination. Addressing these factors can meaningfully improve energy, especially when they are paired with a medical condition that is otherwise well managed.
When hormones are relevant
Hormonal changes can affect energy, sleep, mood, and body composition, but hormone testing is not a one-size-fits-all answer to fatigue. Thyroid evaluation is commonly appropriate because thyroid disorders are a recognized cause of low energy. For some adults, symptoms and medical history may also warrant a conversation about menopause, testosterone concerns, or other endocrine issues.
Any hormone treatment should follow a careful review of symptoms, examination findings, lab results when indicated, medical history, and treatment goals. A personalized plan matters more than treating a number on a lab report.
What happens after the results return
Results are the beginning of the next conversation, not the end of care. If a clear cause is found, treatment may include adjusting a medication, addressing a deficiency, managing blood sugar or thyroid changes, improving sleep, or treating another identified condition. Your provider may recommend repeat testing to make sure the plan is working.
Sometimes the initial results are normal. That does not mean your symptoms are not real. It may mean the next step is to look more closely at sleep, mental health, nutrition, daily routines, or less common medical causes based on your individual symptoms. Follow-up is especially valuable when fatigue persists, worsens, or changes in character.
Small, practical details can help during this process. Before your visit, consider noting when fatigue occurs, how many hours you sleep, your usual meals and caffeine intake, and any associated symptoms. A brief symptom log gives your provider a clearer starting point and can help you notice patterns yourself.
When to seek prompt medical care
Fatigue is often evaluated in a routine office visit, but certain symptoms should not wait. Seek urgent medical attention for fatigue accompanied by chest pressure, trouble breathing, fainting, confusion, sudden weakness, black or bloody stools, or a rapid, unexplained decline in your ability to function.
